INFORMATION SHEET FOR ANNENBERG SCHOOL STUDY

This study has been approved by the USC University Park Institutional Review Board (IRB).

Member Perceptions of a Non-Profit Charity Organization

You are asked to participate in a research study conducted by Jennifer L.Heckman, Doctoral Student and Dr. Margaret McLaughlin, Ph.D. from the Annenberg School for Communication at the University of Southern California.  You were selected as a possible participant in this study because you are a member of the Outreach Commission affiliated with Trinity Episcopal Church.  Eligible subjects to participate will be between the ages of 18 and 80.  A total of 15 subjects will be selected from this group to participate. Your participation is voluntary.

PURPOSE OF THE STUDY We are asking you to take part in a research study because we are trying to learn more about the roles and perceptions of volunteers in a charitable organization.

Completion and return of the questionnaire or response to the interview questions will constitute consent to participate in this research project.

PROCEDURES You will also be asked to complete a brief questionnaire regarding your role in Outreach, your perceptions of the organization, your feelings about leadership and volunteer work in a church outreach program and your ideas about the succes of such groups. You'll also be asked information such as personal characteristics (age, years with Outreach, gender, name).  It will take you approximately 10 minutes to complete the online survey.  There is no cost to you for participating.

POTENTIAL RISKS AND DISCOMFORTS There are no anticipated risks to this study. Completing the questionnaire and survey may be inconvenient or some questions may be embarrassing but you may skip any question you do not like.  You may feel some discomfort by being asked about how you feel about your role within Outreach, your feelings of effectiveness in your role and information regarding your participation.  You may decline to answer any question that makes you feel uncomfortable.

POTENTIAL BENEFITS TO SUBJECTS AND/OR TO SOCIETY 

For yourself:  The possible benefits of your participation in this study are you may become more aware of your own feelings about your role in Outreach and your perceptions of the organization. You may not directly benefit from participating in this research study.

 To others:  The results of this research may be published to inform other charitable organizations and organization members about the role of the volunteers.

 PAYMENT/COMPENSATION FOR PARTICIPATION You will not be compensated for participating in this study.

CONFIDENTIALITY

Any information that is obtained in connection with this study and that can be identified with you will remain confidential and will be disclosed only with your permission or as required by law.

When the results of the research are published or discussed at conferences, no information will be included that would reveal your identity.  You will be assigned a number upon submitting your survey, and this ID number will be stored with your answers to the survey questions.  Your name will not be retained as part of the record.  You have a right to review these records if you wish.  Only the principal investigators and their research assistants will have access to these records.  The data records will be kept in a locked file and destroyed three years after the date of collection.

PARTICIPATION AND WITHDRAWAL You can choose whether to be in this study or not. If you volunteer to be in this study, you may withdraw at any time without consequences of any kind. You may also refuse to answer any questions you donít want to answer and still remain in the study. The investigator may withdraw you from this research if circumstances arise which warrant doing so.

RIGHTS OF RESEARCH SUBJECTS You may withdraw your consent at any time and discontinue participation without penalty. You are not waiving any legal claims, rights or remedies because of your participation in this research study.
